They finally tracked me down for the employee one. 43 questions rate from 1 to 5. Rated 4 for most that should keep them happy.
I don't believe it was anonymous so I was very careful answering the supervisor ones.

That's what I would have done. It's "anonymous" but they know exactly when you completed it. Did you hear if your co-worker who was dizzy is ok?
Boss was not approachable on that and I don't have (the co worker's) number.
